Transfer your health records to Catalpa Clinic
If you're transitioning to Catalpa Clinic from another provider, sharing your MyChart health records helps Dr. Wooldridge review your medical history, current medications, allergies, and past diagnoses before your first visit. This means less time filling out paperwork and more time focused on your care.
What gets shared?
When you share your MyChart data, it typically includes your medical history, problem list, medications, allergies, immunization records, lab results, and visit summaries. Dr. Wooldridge will review this information to provide you with the most informed care possible.
How to Share Your Records
Log into your existing MyChart account
Go to your current healthcare provider's MyChart portal and log in with your existing credentials. This is the MyChart account from your previous or current doctor's office, hospital, or health system.
Find the "Sharing" or "Share Everywhere" option
In MyChart, look for a section called "Share My Record," "Share Everywhere," or "Health Summary" — the exact name varies by health system. It may be under the Menu, Settings, or Account section.
Search for Dr. Bryan Wooldridge
Use the search function to find Bryan Wooldridge as the receiving provider. The specialty will be listed as Family Medicine. Note: search by the doctor's name, not the clinic name.
If you're unable to find Dr. Wooldridge through the search, you can use his direct address instead:
Copy and paste this into the "Direct Address" or "Send to a provider" field in your MyChart sharing settings.
Please note: This is a Direct Address used exclusively for secure electronic health record (EHR) transfers. It is not an email address and does not accept traditional email. Only messages originating from a certified EHR system (such as MyChart, Epic, or Cerner) will be received.
Confirm and send
Review the information being shared, then confirm. The records will be transmitted securely and will appear in your chart at Catalpa Clinic, typically within a few business days.
Alternative: Download and Bring Your Records
If you're unable to share electronically, you can download your health summary from MyChart as a PDF or print it, and bring it to your first appointment. Dr. Wooldridge and his team will enter the key information into your chart.
