Online scheduling is provided as a convenience through our third-party scheduling platform (Elation Health). Submitting a scheduling request does not guarantee an appointment. Catalpa Clinic reserves the right to accept or decline new patients at our discretion. All new patients must be ages 14 and older.
4. Insurance and Payment
While we display the insurance plans we commonly accept on our website, coverage verification is ultimately the responsibility of the patient. Listing an insurance provider on our website does not guarantee that your specific plan will be accepted. We recommend calling our office at (360) 362-0170 to verify coverage before your visit.
Patients are responsible for any co-pays, deductibles, coinsurance, and services not covered by their insurance plan. Payment is expected at the time of service unless other arrangements have been made in advance.
5. Prescription Policy
As stated on our website, Dr. Wooldridge does not prescribe controlled substances. Treatment plans for conditions such as anxiety, depression, insomnia, ADHD, and chronic pain utilize non-controlled therapeutic options. This policy is firm and is not negotiable on a case-by-case basis. Referrals to appropriate specialists are available for patients requiring controlled substance management.
